Unit/Organization Plaque Request
In honor of an outstanding unit or organization of the military or supporting the military.

Plate Instructions: Plaques must be approved by the All Veterans Memorial Committee and the City of Emporia prior to placement. Placement of the plaque at a desired location cannot be guaranteed but will be considered.
- Plaques are ordered and purchased by the AVM Committee using the information you provide and this form.
- Please sketch and write clearly your plaque design. All plaques are rectangle in shape.
- Please attach the drawing/draft example of your plaque. If using any images/symbols please include high definition copies.
- Send this form and your design to the address below.
- Our vendor/designer will produce a proof and estimated cost that we will share with you.
- Once all parties approve the plaque proof and you submit your payment we will order and place your plaque.
